Why WhatsApp reminders work in Kenya

Most of your patients already open WhatsApp many times a day. An SMS can feel like an advert; a WhatsApp message from a clinic they know feels like a conversation. It can also carry a reply, which is the whole point. A reminder that lets a mother answer "1 to confirm, 2 to reschedule" tells your team who is coming before the clinic day starts.

A reminder is only useful if someone reads the replies. Before you send the first message, decide who in your facility checks replies each morning and what they do with a "2" or a worrying answer.

Six rules for a reminder patients answer

  • Name the clinic in the first line. Patients ignore messages from numbers they cannot place.
  • Send twice. Two days before gives time to plan transport and money; the morning of the visit catches the ones who forgot.
  • Always offer a simple reply. "Reply 1 to confirm or 2 to reschedule" gets far more answers than "please confirm your attendance".
  • Keep clinical details out. Never put a diagnosis, test result or HIV status in a reminder. Phones are shared.
  • Use the language the patient chose. Ask at registration: English or Kiswahili. Record it and stick to it.
  • Give a way out. End with "Reply STOP to stop these messages" and honour it the same day.

5. Post-visit check-in (day 3 to 5)

Kiswahili

6. Opt-out line (add to the end of any template)

English

Kiswahili

Jibu STOP kusitisha jumbe hizi.

Any reply of "3" or a message describing bleeding, severe pain or other warning signs must reach a nurse the same day. A reminder system without a clinical escalation path is a risk, not a service.

Phone numbers and visit details are personal data, and anything about a patient's health is sensitive personal data under Kenya's Data Protection Act, 2019. In practice that means three things: ask for consent to message at registration and record it, send only what the reminder needs, and stop immediately when a patient opts out. If you use the WhatsApp Business Platform, the first message in a conversation must use a template that Meta has approved, so plan your wording before you start. Our patient data protection checklist covers the rest.

When manual reminders stop scaling

A committed nurse can send and track reminders for a few dozen patients a week. Past that, messages go out late, replies sit unread, and the patients who go quiet are the ones nobody notices. That is the point where a system earns its keep: it sends on time, reads every reply, and hands the concerning ones to a person.

That is what HealthBridge MCH does for mothers and babies, from the first ANC contact to the last immunisation at 18 months, in English or Kiswahili.
